Enliven Initiative

The Enliven Initiative was launched in 2018 as a transformative effort to revitalize Bartz-Altadonna Community Health Center's culture and operational approach. Recognizing the need for deeper alignment between our mission and daily practices, the initiative was designed to foster a value-driven, patient-centered environment while improving clinical quality and organizational performance.

At its core, Enliven focuses on:

1. Values Formation & Alignment: Reinforcing our core values of compassion, respect, integrity, teamwork, and accountability across every level of the organization.
2. Cultural Sensitivity: Ensuring our workforce is equipped to understand and respond to the diverse needs of our patient population.
3. Patient Quality Outcomes: Centering patient health outcomes and experience as key measures of success.
4. Data-Driven Performance: Using the Patient-Centered Medical Home (PCMH) model, combined with the Objectives, Goals, Strategies, and Measures (OGSM) framework, to drive decision-making, enhance care coordination, and track performance improvements.
